Reality UnravelerPassive
All terrain within 1 mile transforms randomly each hour. Gravity, direction, and physical laws may change. Structures collapse. Time loops.
Nightmare Tier — Godforged Entity
“When all worlds end, I remain.I am the echo of everything.”
Azraelion is a convergence — born from the collective dread of countless worlds, shaped by the darkest myths, gods, and final bosses across realms. It is no longer a being, but an event. Its presence warps reality, memory, and fate.
Just being within 1 mile of Azraelion causes the following persistent effects:
All terrain within 1 mile transforms randomly each hour. Gravity, direction, and physical laws may change. Structures collapse. Time loops.
Azraelion strikes every enemy it can see. Each target must succeed on a DC 30 save or be hit with 10d12 force + 10d12 necrotic damage and be banished to a random plane for 1 round.
Creatures within 300 ft. must succeed on a DC 28 Wisdom save or suffer 10d10 psychic damage and have their maximum HP reduced by the same amount until long rest.
Azraelion rewrites a 60 ft. cube of existence. Spells are countered. Memories are altered. Allies may forget their purpose.
Resurrection magic, divine intervention, and wish-type effects have a 80% chance to fail within Azraelion's presence.
Summons 1d4+1 Harbingers of the End or lesser nightmare entities.
Balanced and wrathful. Reality begins to distort.
Loses form. Space and time become fluid.
Becomes incorporeal. Only force and radiant can harm.
Exists in all places. Attacks ignore distance or cover.
Does not die. Becomes a permanent part of the world.

What the people of Aetherion said when Azraelion came — from carved steles to warden comms.
Translated from carver-marks. The stele lists worlds by the way they ended.
Another world ended tonight. I heard it the way you hear a door close in a far room.
Every ending leaves a residue. Fear that has nowhere left to go.
And where does it go?
Here. It is gathering. Carve this deep: when enough dread gathers, it stops being a feeling and becomes an event.
Field log from the first place Azraelion was recorded. The district is still a Nightmare Zone today.
Compass is spinning. My watch says 02:17. It said 02:17 an hour ago.
Spell lattices are misfiring across the whole ward. Ring four turned to glass.
The dead in the old cemetery are talking. Not moving — talking. They are telling us what happens next.
Then tell me what they say.
"When all worlds end, I remain." Every one of them. Same words.
Pull everyone back past the mile line. Whatever this is, it hasn't even arrived yet.
Council minutes declassified after the second Convergence.
Our Ascendants brought it to nothing. Our scribes watched it reach zero. It did not die.
It transcended. Every time we 'win' it comes back whole, and the ground it stood on never heals.
Then we stop calling it a war. It is weather. We map where it falls and we build where it doesn't.
Record it: Azraelion is not to be defeated. It is to be endured. All kingdoms will hold the line together.
Open comms from a raid party. Callsigns preserved.
It's losing its shape. The armor's gone, it's just… smoke and eyes.
That's the Unraveling. Floor's about to stop being a floor. Everyone off the stone!
Only force and radiant are landing. Switching to Solar.
Okoye. I can't remember why we came in here.
Void Rewrite. Eyes on me, Pell. We came to hold the line. Breathe. Say it back.
…Hold the line.
Overheard by a route scribe of the Caravaners Union.
Grandfather, why don't the Ascendants just kill it?
They did, child. Six times. Each time it ended, a new Nightmare Zone was born where it fell.
That's why the routes bend. Every bend on the map is a place Azraelion has 'ended'.
And someone found a Shard of Finality in the last one. Wished his daughter back. She came back. She never sleeps now.
Transcribed by the Aetheric Script Translator from six Zones at once.
You call me a monster. I am only what your worlds were afraid of, gathered in one place.
Every ending you survive, you carry here. I am the echo of everything.
Come, Ascendants. You will not defeat me. Let us see how long you endure.
“You don't defeat Azraelion. You endure it.And even that… might not be enough.”
